It is always our challenge to effectively communicate our policies to our patients. We thought this would be a good forum. Please let us know if you have questions on any of our current policies and procedure.
Identification - We will be asking for identification at check-in for the prevention of identity theft. Though we may know some of our patients; we are asking all patients to scan a drivers license or state issued identification card. This procedure complies with the Federal Trade Commission Red Flag Policy. Thank you for your cooperation.
Under Age 18 - Patients presenting for an appointment under the age of 18 must be accompanied by a parent or have a note giving permission to treat. Parents must be present for patient's initial visit.
Power of Attorney - If you have a power of attorney, you must present the office with a copy.
Late Appointments - If a patient arrives 0 - 10 minutes late for a scheduled appointment, we will fit them in, in the order they arrive. If they are greater than 10 minutes late we will accommodate them into the schedule. If patient is more than 20 minutes late we will kindly ask patient to reschedule.
Missed Appointment - Failure to show-up for an appointment could result in a $25 fee.
Cancelled Appointments - We kindly ask for a 24-hour cancellation notice.
Insurance - A patient without insurance will be required to pay in full upon leaving.
As always, it is the patient's responsibility to contact insurance carrier to verify coverage for services such as wellness exams, physicals, immunizations, outside testing, labs etc.
Copays- Copays are due at time of service.
Cosmetic Procedures - We require payment in full for all cosmetic procedures at time of service.
Patient Due Balances - We require payment on the patient due portion of the bill upon checking out.
Copies of Records - Please allow 1 week for processing and there is a fee for medical record copies.
Refill Requests - Please allow 48 hours for processing refill requests.
Disability or Other Insurance Forms - Please allow up to 72 hours for processing. There may be a fee depending on the length and time to complete the form.
